Won't you join us for a whole day of fun to help support WCCA and our Meals on Wheels Program? Every dollar we raise from this event helps support our programs which provide hot meals to the seniors in Wise County that need it the most. With over 34,000 meals delivered a year and counting, WCCA needs you now more than ever so that we may continue providing meals to those that are in need. Tournament Pros Putting Contest Agreement (PLEASE READ FOR YOUR OWN PROTECTION!!) COPY AND SEND THIS TO THE GOLF COURSE!

PAYMENT: Payment must be made to Tournament Pros (TP) prior to the event unless other billing arrangements have been made. By making payment you agree to the terms & conditions (T&C) of the contract.

PRIZE: Unless otherwise noted in the contract, the stated prize will be awarded for ONE winning putt during the event provided all the T&C are followed. Unless otherwise stated, the “winning” player must not be or ever have been a professional.

WITNESSES: It is the sole responsibility of the tournament to provide ONE (1) non-playing adult (age 18 or older) AND have someone from the golf course witness the Final Putt. The witnesses must be in position to see the golfer strike the putt and watch the ball through the entirety of the putt until it reaches the hole. In the event of a winner, the witnesses will be asked to describe the entire shot.

THE WITNESSES SHOULD BE TOLD THAT A CLAIM MAY BE DENIED IF THEY DO NOT SEE THE ENTIRE SHOT! A VIDEO OF THE FINAL PUTT SHOWING THE STRIKE AND FOLLOWING THE BALL ALL THE WAY TO THE HOLE MUST BE SUBMITTED TO VERIFY ANY CLAIM(S).

DISTANCE: The initial measuring of the required distance of the Final Putt must be done by PGA professional or assistant professional. The required distance of the putt as indicated in the contract is measured from the point the ball will be struck, to the hole. Any "winning" claim that results from a putt taken from a distance LESS than the stated requirements may be denied.

ATTEMPTS: Only one stroke is permitted for the final prize unless otherwise confirmed in writing by TP. No practice putts, second attempts or mulligans that went in the hole would be awarded any prize. The player attempting the final putt CANNOT hit any practice putts toward the final hole, nor roll a ball on the intended line, nor have anyone else putt or roll a ball on the intended line of the final putt. Pin must be pulled for final putt. If the player sees a ball roll on the intended line through any of these means, the line of the final putt must be moved at least twenty feet.

MULTIPLE FINAL PUTTS: In the event that there is more than one Final Putt (which must get prior approval by TP), the same Target Hole can be used for each additional Final Putt as long as each putt is struck from no closer than 15 feet from the previous attempt, i.e. so subsequent putters cannot see which way the putt breaks.

ELIMINATION ROUNDS: In the course of determining the player(s) who will attempt the Final Putt(s)…the “Elimination Round(s)” putt(s) shall be less than 25 feet AND not using the final 50 foot putt hole*. *If no other hole is available, any elimination round putt(s) must be taken from a spot at least 25 feet from starting spot where final 50 foot putt will be attempted from.

CLAIMS: In the event of a winner, TP must be notified no later than the next business day after the event or the claim may be denied. The tournament agrees to cooperate fully during the verification process and return the requested documentation (forms will be emailed, faxed or mailed to the tournament organizers) within four weeks of the event. (The claim may be denied if forms are received more than four weeks after the tournament.) Winner(s) will be responsible for taxes on the value of the prize. A W9 Form and Release letter will be sent to the winner(s) to fill out and must be returned before any prizes are awarded.

arrangements have been made. By making payment you agree to the terms & conditions (T&C) of the contract.

PRIZE: The stated prize (in contract) will be awarded ONCE for a “winning” shot (closest to the Pin within 12") or divided among multiple winners (Closest to the Pin within 12"). Unless otherwise stated, the “winning” player(s) must not be or ever have been a professional. A “winning” shot must be on a par three hole on a regulation golf course. Constructed holes, temporary greens or modified par fours and fives are not eligible unless approved by TP prior to the event. Only one pre-designated cup may be used. All pins and cups must conform to USGA standards and specifications.

WITNESS(ES): It is the sole responsibility of the tournament to provide at least one (1) non-playing adult (age 18 or older), to be a witness on the prize hole. The witness(es) must be in position to see both the teeing area (i.e. the golfer's swing) and the hole (base of the pin flag). In the event of a “winning” shot, the witness(es) will be asked to describe the entire shot. THE WITNESS(ES)

SHOULD BE TOLD THAT A CLAIM MAY BE DENIED IF THEY DO NOT SEE THE SWING AND THE BALL COME TO REST AND MAKE REQUIRED MEASUREMENT (WITH A TAPE MEASURE OR RULER).

YARDAGES: The necessary yardage requirements for the Par 3 hole(s) (for men, including seniors) are detailed in the contract.

The required distance is measured from the tee-markers to the hole. No one can be closer than 165 yards with the exception of women who are permitted to play from the women's tee markers as long as they are not closer than 15 yards from the men's tees. Any “winning” claim that results from a shot taken from a distance LESS than the listed yardage requirements for each hole may be denied. (Please have the golf course REMOVE all tee-markers from the Par 3’s other than those designated for the required distances in the contract.)

MEASUREMENT: This must be done by the witness or from someone specifically assigned by the Tournament Director. It is to be measured from the nearest edge of the ball where it comes to rest to the nearest edge of the cup using a standard tape measure, ruler or yardstick.

ATTEMPTS: Only one swing per player per hole is permitted for a “winning” shot unless otherwise confirmed in writing by TP. No practice shots, second attempts or mulligans would be awarded any prize. (On nine-hole courses, a player is eligible to win a stated prize only the first time he/she plays the hole.)

CLAIMS: In the event of a “winner”, TP must be notified no later than the next business day after the event or
the claim may be denied. The tournament agrees to cooperate fully during the verification process (forms will be emailed, faxed or mailed to the tournament organizers) and return the requested documentation within four weeks of the event. The claim may be denied if the forms are received by TP more than four weeks after the tournament. Winner(s) will be responsible for taxes on the value of the prize.

A W9 Form and Release letter will be sent to the winner(s) to fill out and must be returned before prizes are awarded.

WEATHER: In the event a tournament is canceled due to weather, TP agrees to refund all fees or change the tournament coverage to another date without charge or penalty if notified via fax, phone or email by the next business day after the event. In the event a tournament is canceled because of weather after play has begun, the fee may be considered earned if 10% or more of the field has played the prize hole.
